Sleep Apnoea & Snoring Treatment

If you're waking up tired despite a full night's sleep, snoring loudly, or your partner has noticed you stopping breathing during the night — these are signs that something is interfering with the quality of your sleep. And for many people, the solution is closer than they think.

At DentalCare Carnegie, we run an in-house sleep clinic offering everything from diagnosis through to treatment — including custom oral appliances that are often the most comfortable and effective solution for snoring and mild to moderate obstructive sleep apnoea.

What Is Sleep Apnoea?

Obstructive sleep apnoea (OSA) is a condition in which the airway repeatedly collapses or becomes partially blocked during sleep, causing breathing to pause — sometimes dozens or even hundreds of times per night. Each pause briefly wakes the brain to restart breathing, even if you're not consciously aware of it. The result is fragmented, unrestorative sleep, no matter how many hours you spend in bed.

OSA is more common than most people realise — and it's significantly underdiagnosed, because many people attribute the symptoms to stress, poor lifestyle, or simply "not being a morning person."

Left untreated, sleep apnoea is associated with serious long-term health consequences including high blood pressure, increased risk of heart disease and stroke, type 2 diabetes, and impaired cognitive function. It also affects mood, concentration, and daily performance in ways that accumulate quietly over time.

Signs You May Have Sleep Apnoea

You don't need to have every symptom — one or two of these is enough to warrant an assessment:

  • Loud or persistent snoring
  • Waking unrefreshed despite adequate sleep
  • Excessive daytime tiredness or difficulty staying awake
  • Morning headaches
  • Waking with a dry mouth or sore throat
  • Difficulty concentrating or remembering things
  • Irritability or low mood without a clear cause
  • Your partner has observed you stop breathing, gasp, or choke during sleep
  • Needing to get up frequently during the night to urinate

Many patients have been living with these symptoms for years — often normalising them or assuming nothing can be done. If this sounds familiar, it's worth talking to us.

Snoring — More Than Just a Noise Problem

Snoring on its own — without sleep apnoea — is extremely common and can still significantly disrupt sleep quality for both you and your partner. It occurs when the soft tissues at the back of the throat partially obstruct airflow, causing them to vibrate as air passes through.

Snoring and sleep apnoea exist on a spectrum. Snoring doesn't always indicate apnoea, but it is one of its most consistent warning signs — which is why it's always worth having it assessed rather than dismissed. For many snorers, a custom oral appliance resolves the problem entirely.

How We Diagnose Sleep Apnoea — Take-Home Sleep Study

You don't need to spend a night in a sleep laboratory to get a diagnosis. At DentalCare Carnegie, we coordinate home sleep apnoea tests (HSATs) — diagnostic devices you use in your own bed, in your own home.

The process is straightforward. You collect a small portable monitor from a local clinic, wear it while you sleep, and return it the following morning.

The device records key measurements overnight including:

  • Breathing patterns and pauses
  • Blood oxygen levels
  • Heart rate
  • Airflow through the nose
  • Chest and abdominal movement
  • Snoring sounds

The data is then analysed by a sleep physician, who provides a diagnostic report. If sleep apnoea is confirmed, the severity is classified — mild, moderate, or severe — which guides the treatment recommendation.

Home sleep studies are accurate, convenient, and considerably less expensive than an in-lab study. They are also far more representative of your typical sleep, since you're in your own environment rather than a clinical setting.

Treatment — Custom Oral Appliances

Treatment — Custom Oral Appliances

For patients with snoring or mild to moderate obstructive sleep apnoea, a custom-made oral sleep appliance is often the most effective and most comfortable treatment available — and for many patients, it's life-changing.

How it works

The most widely used type is a mandibular advancement device (MAD) — an appliance similar in appearance to a mouthguard that fits over both upper and lower teeth. It works by gently holding the lower jaw slightly forward during sleep, which opens the airway and prevents the soft tissue collapse that causes snoring and breathing pauses.

Because it's custom-made from precise impressions of your own teeth, it fits comfortably and securely — a significant difference from over-the-counter devices, which are bulky, imprecise, and often abandoned within weeks.

Who it suits

Custom oral appliances are appropriate for:

  • Snorers without diagnosed apnoea
  • Patients with mild to moderate OSA
  • Patients who have tried CPAP and found it intolerable
  • Patients who travel frequently and need a portable alternative to CPAP
  • Patients with mild apnoea who want a less intrusive solution

For severe OSA, CPAP therapy remains the gold standard — but oral appliances can be used alongside CPAP in some cases, or as an alternative where CPAP compliance is genuinely poor.

The advantages of oral appliance therapy

The advantages of oral appliance therapy

Silent and unobtrusive — no mask, no machine, no noise

  • Small and portable — fits in a pocket, ideal for travel
  • High compliance — patients wear them consistently because they're comfortable
  • No power source required
  • Easy to maintain

CPAP vs Oral Appliances — Which Is Right for You?

CPAP

CPAP (continuous positive airway pressure) is the most effective treatment for severe OSA — it delivers a continuous stream of pressurised air through a mask to keep the airway open throughout the night. For patients with severe apnoea, it's typically the first recommendation.

However, CPAP compliance is a significant clinical challenge. Many patients find the mask uncomfortable, claustrophobic, or disruptive — and studies consistently show that a meaningful proportion of CPAP users either abandon it or use it inconsistently, leaving their apnoea undertreated.

Oral Appliances

For mild to moderate OSA, custom oral appliances have been shown to achieve comparable clinical outcomes to CPAP — with substantially better compliance. A treatment that's comfortable enough to actually be used every night outperforms a theoretically superior treatment that sits in the bedside drawer.

We work collaboratively with sleep physicians and can communicate with your GP or respiratory physician to ensure your treatment is coordinated and appropriate for your specific diagnosis.

What Does the Process Involve?

What Does the Process Involve?

Step 1 — Consultation
You come in to discuss your symptoms. We review your sleep history, assess your teeth, jaw, and airway, and determine whether a home sleep study is appropriate or whether a referral to a sleep physician is the better starting point.

Step 2 — Home Sleep Study
If indicated, we coordinate a home sleep apnoea test. You collect the device from a local clinic, use it at home overnight, and return it the next day. Results are typically available within one to two weeks.

Step 3 — Review and Treatment Planning
Once your results are in, we review the findings with you and discuss your treatment options. If a custom oral appliance is appropriate, impressions or digital scans of your teeth are taken.

Step 4 — Appliance Fitting
Your custom appliance is made and fitted at your next appointment. We make any adjustments needed for comfort and show you how to use and care for it. Most patients adapt within the first week or two of wearing it.

Step 5 — Follow-up
We monitor your progress and can make incremental adjustments to the appliance to optimise its effectiveness over time. For patients where a follow-up sleep study is warranted to confirm treatment efficacy, this can also be coordinated.

Caring for Your Sleep Appliance

Caring for Your Sleep Appliance

With proper care, a custom sleep appliance typically lasts 3–5 years. To keep it in good condition:

  • Rinse it with cold water before and after use
  • Clean it daily with a soft toothbrush and mild soap — avoid toothpaste, which is abrasive
  • Store it in its protective case when not in use, away from heat
  • Bring it to your regular dental appointments so we can check its condition and fit

Frequently Asked Questions

Yes — dentists play an important role in the diagnosis and treatment of snoring and mild to moderate obstructive sleep apnoea.

A mandibular advancement device (MAD) is a custom-made oral appliance, similar in appearance to a mouthguard, that fits over your upper and lower teeth. It works by gently holding your lower jaw slightly forward during sleep, which opens the airway and prevents the soft tissue collapse that causes snoring and breathing pauses.

We refer you for a home sleep apnoea test (HSAT) — a small portable monitor you wear overnight in your own bed. The device measures your breathing patterns, blood oxygen levels, heart rate and airflow while you sleep. Your results are analysed by a sleep physician, who provides a diagnostic report, usually within one to two weeks.

Not necessarily — you can come directly to us. We'll assess your symptoms and history, and if appropriate, refer you for a home sleep study. If your results indicate you need further input from a sleep physician or respiratory specialist, we'll work collaboratively with those practitioners to ensure your care is coordinated.

You're not alone — CPAP non-compliance is a significant clinical challenge. Many patients find the mask uncomfortable or claustrophobic and abandon it. For mild to moderate OSA, custom oral appliances have been shown in clinical studies to achieve comparable outcomes to CPAP, with far better compliance. A treatment that's comfortable enough to be worn every night outperforms one that sits in the drawer. We'd be happy to discuss whether an oral appliance is a suitable option for you.

Pharmacy mouthguards are one-size-fits-all and made from soft material that doesn't position the jaw with the precision needed to open the airway effectively. A custom appliance is made from precise impressions of your teeth, fits securely, can be incrementally adjusted for optimal effectiveness, and is far more comfortable to wear consistently.

With proper care, a custom sleep appliance typically lasts 3–5 years. We'll check the condition and fit of your appliance at routine appointments and advise when it needs adjustment or replacement.
